fix bin/gen_top_html

Fix `bin/gen_top_html` to generate the proper Nu HTML check URLs.

Changed `bin/gen_top_html` VERSION from "1.3 2024-04-14"
to "1.4 2024-04-15".

Remove improper "xmlns:cc="http://creativecommons.org/ns#"
from `inc/inc/footer.default.html`.

Run `make gen_top_html` to perform the above and test under macOS.

TODO: If this is successful, rebuild all other constructed HTML files.
